Getting the treatment you need should be an affordable, accessible, and stress-free process. We participate in a large list of insurance carriers so that this is possible. This list is regularly growing and changing so that we may best serve our patients and the communities’ needs.

Frequently Asked Questions

Do I need a prescription for physical therapy?

Under direct access, you can receive physical therapy for six visits or up to 30 days without a prescription from a physician. Contact one of our offices to determine if your insurance company allows physical therapy benefits without a prescription.

Do you accept my insurance?

We participate in a large list of insurance carriers which continues to change and grow to service the communities’ needs. Contact one of our offices to see if we participate with your insurance company. As a courtesy to our patients, we verify insurance coverage and benefits prior to your initial evaluation.

What should I bring to my appointment?
Arrive 15 minutes before your initial appointment to complete paperwork. You can find our forms above. We require your driver’s license (or another form of picture identification) and your insurance card. In addition, bring any prescriptions for physical therapy or reports of your medical condition received from your physician.
What should I wear to my appointment?
Wear clothing that is loose and comfortable. If you use any assistive devices such as a brace, splint or crutches, be sure to bring them to your visits.
How long will my appointment take?
You should plan on being at your appointment for approximately an hour. An initial evaluation may take approximately one and a half hours. If you are a new patient, please arrive 15 minutes before your scheduled appointment to complete paperwork.
Do you communicate with my doctor?
Yes, we communicate with your doctor. After your initial evaluation, we send a plan of care to your primary physician or to the physician who referred you to Quinnipiac Physical Therapy & Sports Medicine. During your treatment, we send progress notes to your primary physician or to the physician who referred you to us.
